Our aim is to promote your independence through individualised Occupational Therapy advice and support.  We work in partnership with you to achieve your goals and maximise your quality of life.

Falls Risk and Home Safety:

Have you had a fall, or near fall recently, or have concerns about your mobility at home?

If so, our Occupational Therapists are able to undertake a Falls risk and home safety assessment. This assessment will identify potential hazards within your home and propose recommendations for managing them - all aimed at ensuring that you maintain your independence and are safe in your own home.

As part of this assessment, we will identify any home modifications and/or equipment (grab rails / etc) that will increase your safety at home. Our Occupational Therapists will take the time to consult with you through out the process. They can organise quotes for home modifications or equipment and help you through the purchasing process (if required).

Activities of Daily Living (ADL) assessment:

Our Occupational Therapists can undertake an Activities of Daily Living assessment to determine the level and type of assistance someone may need in order to live independently.

ADL assessments, determine a clients ability to undertake the following activities (note: the below is not exhaustive):

  • Money management
  • Personal care
  • Management of medications
  • Cleaning
  • Cooking and food preparation
  • Mobility and ability to transfer

After the assessment the OT will prepare a report that contains recommendations on support needs, assistance technology and/or home modifications. We can also assist with implementing recommendations and can provide information about different Government funding options. We support you throughout the process and will make sure you, and your loved ones understand the contents of the report and the next steps.

Home modifications, equipment and assistance technology:

Our OT's have extensive experience in the assessment and prescription of home modifications, equipment and assistance technology. Typical home modification, equipment and assistance technology include:

  • Internal / ramps.
  • Walking aids, mobility scooters, wheelchairs, shower chairs, toilet seat raisers, beds, low risk assistance equipment (utensils, modified kitchen aids, etc), and electric lift recliner chairs (to name a few).
